Treating Male Pattern Hair Loss When Case is Too Advanced and Young for Transplants

A 21-year-old gentleman explains that he has lots of hair loss. He wants to know if hair transplantation would be a good idea for him now, or when he turns 22.

Dr. Amiya Prasad is a board certified cosmetic surgeon practicing in Manhattan and Long Island for over 20 years. Hair restoration is a significant part of his practice. There are two drugs that are used for male pattern hair loss in the United States: minoxidil and finasteride. Minoxidil which is a topical is mostly likely not to be of too much value for him. Finasteride is the drug that inhibits the enzyme called 5-alpha reductase. This is an enzyme responsible for the conversion of testosterone to dihydrotestosterone (DHT) which is one of the major causes of hair thinning in males. This only really works in men who have DHT-sensitive hair follicles as DHT is not the only cause of hair thinning. Many times, younger people who have this aggressive hair loss do not respond to finasteride. In addition, finasteride is not everyone’s favorite drug because of concerns of long-term sexual side effects that were published in journals.

In Dr. Prasad’s practice, he has had success with a treatment called Hair Regeneration which is his method of formulation of a material called Acellular matrix combined with platelet-rich plasma (PRP) derived from the patient’s own blood. He has 3-5 years of data that shows close to 100% improvement in male pattern hair loss. The reality is that the more advanced someone’s hair loss is, the less volume of hair he is able to get back. However, in patients who are younger, he has had some remarkable results even in people Dr. Prasad didn’t anticipate to get such good results. In some patients, he also does a second treatment which he does approximately after a year or between 12-18 months.

Dr. Prasad follows his patients very closely every 3 months. He does microscopic photos, digital photography, andlooks at the data very critically. When someone has a mismatch, eventually they will lose the existing hair and all that’s left are transplants. Transplanted hair doesn’t look very good, especially for a younger man.
