A 21-year-old gentleman explains that he has lots of hair loss. He wants to know if hair transplantation would be a good idea for him now, or when he turns 22.
Dr. Amiya Prasad is a board certified cosmetic surgeon practicing in Manhattan and Long Island for over 20 years. Hair restoration is a significant part of his practice. He is also the founder of Trichostem® Hair Regeneration centers and has been very well known for developing a procedure called Hair Regeneration which has been very successful. He has patients who come to him from all over the world for this revolutionary hair loss treatment.
Dr. Prasad explains that this gentleman has very advanced hair loss and he is losing hair very quickly, so a hair transplant is not likely to be successful for him. There is a mismatch with the degree of hair loss he has and the amount of hair available for transplantation. The aggressiveness of his hair loss pattern is too great for a transplant procedure.
There are two drugs that are used for male pattern hair loss in the United States: minoxidil and finasteride. Minoxidil which is a topical is mostly likely not to be of too much value for him. Finasteride is the drug that inhibits the enzyme called 5-alpha reductase. This is an enzyme responsible for the conversion of testosterone to dihydrotestosterone (DHT) which is one of the major causes of hair thinning in males. This only really works in men who have DHT-sensitive hair follicles as DHT is not the only cause of hair thinning. Many times, younger people who have this aggressive hair loss do not respond to finasteride. In addition, finasteride is not everyone’s favorite drug because of concerns of long-term sexual side effects that were published in journals.
In Dr. Prasad’s practice, he has had success with a treatment called Hair Regeneration which is his method of formulation of a material called Acellular matrix combined with platelet-rich plasma (PRP) derived from the patient’s own blood. He has 3-5 years of data that shows close to 100% improvement in male pattern hair loss. The reality is that the more advanced someone’s hair loss is, the less volume of hair he is able to get back. However, in patients who are younger, he has had some remarkable results even in people Dr. Prasad didn’t anticipate to get such good results. If this gentleman were to go to Dr. Prasad’s practice, he would recommend doing the injection treatment as soon as possible.
Unfortunately, there are some people who don’t respond to the treatment and Dr. Prasad needs to figure out why that’s the case. He has done reinjections in those patients, but he still hasn’t gotten responses. Fortunately, for the male pattern population that he has treated, close to a 100% have had responses where they have seen a cessation or stopping of the hair loss and thickening of their thinning hair. In some patients, he also does a second treatment which he does approximately after a year or between 12-18 months.
Dr. Prasad follows his patients very closely every 3 months. He does microscopic photos, digital photography, andlooks at the data very critically. He has been so successful with this that he has now scaled it with a company he started called Trichostem® Hair Regeneration Centers. He has been able to intervene at an early stage and avoid the unfortunate cost and disappointment of hair transplants. When someone has a mismatch, eventually they will lose the existing hair and all that’s left are transplants. Transplanted hair doesn’t look very good, especially for a younger man.
Dr. Prasad suggests that this gentleman learn more about Hair Regeneration, and also think about this in context of where he is at now. He has to accept a certain amount of hair loss because that’s a normal part of his look, but treatment is something that he has to decide. Dr. Prasad’s recommendation as a doctor and as someone who understands the benefits of having good hair is that he must intervene. If he can stop the progression of hair loss and restore some volume, that would be a great solution.
